Americans with Disabilities Act (ADA) Consultation
Provides technical assistance and support to individuals and businesses to ensure ADA accessibility in the community.

BENEFIT SERVICES

ACCES-VR Orientations
Delivers orientation to individuals applying for ACCES-VR vocational rehabilitation services.

Benefits Advisement
Provides person-centered support to help individuals understand and maximize benefits and work incentives.

Facilitated Enrollment
Provides outreach and assistance to individuals who are underinsured or uninsured, helping them enroll in Medicaid, Medicare, and related benefits.

Supportive Housing
Provides housing subsidies and case management in partnership with Safe Harbors of the Hudson.

Consumer Directed Personal Care Program (CDPAP)
An alternative to traditional at-home care for persons with disabilities and older adults, enabling them to hire and manage their own personal aides.

Deaf and Hard of Hearing Services
Advocates for the rights and needs of Deaf and Hard of Hearing individuals, provides support services, and facilitates access to resources and opportunities.

EMPLOYMENT & FAMILY SERVICES

Community Independence Program (CIP)
Provides person-centered, skill-building day habilitation in a group setting where participants choose their recreational and community activities. Transportation provided. (An OPWDD Program)

Education Advocacy
Provides information, referrals, and assistance to families and professionals to support the educational rights of students with disabilities.

Independent Living Skills Services (ILSS)
Provides customized support to help individuals with disabilities enhance their independence and access essential resources and services.

Learning Enrichment Afterschool Program (LEAP)
Offers afterschool recreation, social skills development, and homework support for students. Operates on school days with parent pick-up by 6:00 PM. (An OPWDD Program)

Pathways to Employment (PTE)
Supports individuals with little or no work experience in exploring employment through job shadowing, internships, and training. (An OPWDD Program)

Supported Employment Program (SEMP)
Assists individuals in obtaining and maintaining competitive employment in the community. (An OPWDD Program)

Enhanced Welcome Orange Geriatric Initiative (EWOGI)
Works with individuals to identify items or home modifications needed to age in place safely within their community.

RECOVERY COMMUNITY OUTREACH CENTER (RCOC)

Reach One
Provides peer support, recovery coaching, and access to vocational, educational, and social resources for adults impacted by mental health conditions.

Onward Recovery
Offers individualized wellness plans, recovery groups, harm reduction, social and vocational support, and connections to community resources.

NY Connects No Wrong Door
Collaborates with the Office for the Aging to help aging individuals with disabilities remain in their communities by identifying necessary supports and services.

OPWDD Housing Subsidy and Supports
Supports individuals who contribute 30% of their income toward rent by managing the subsidy for the remaining cost. Also manages budgets and provides technical assistance and support. (An OPWDD Program)

PEER SERVICES AND CRISIS RESPONSE

Diversion – 988/311
Connects individuals in crisis (referred by 988/311 or the Mobile Response Team) with one-on-one, person-centered supports and services.

Forensic Court Connections
Offers court-based diversion to treatment with the goal of reducing recidivism and supporting recovery and stability.

Hospital Bridger
Provides essential support to individuals still under hospital care to ensure a successful return to the community and reduce remissions.

Intensive and Sustained Engagement and Treatment (INSET)
Provides intensive peer support to individuals at risk of requiring court-ordered treatment.

Peer Parent
Delivers peer support for parents of children experiencing mental health challenges, including crisis support to help prevent school displacement.

State Opioid Response (SOR)
Supports adults with substance use disorders through intensive peer support, social and recovery activities, harm reduction, and vocational/educational support.

Transition Support and Peer Advocacy
Supports young adults in accessing benefits, services, and supports to build independence. Specializes in working with individuals aging out of foster care or school systems.

Self-Directed Care
Offers a person-centered planning process that allows individuals greater control over their care funding, promoting flexibility and independence.

Statewide Systems Advocacy Network (SSAN)
Collaborates with Independent Living Centers across New York State and other disability organizations to advocate for legislation that benefit people with disabilities.
